Please Note, the University no longer accepts physical copies of documents, including transcripts. All documentation should be submitted online via your Application Checklist.
Additional Materials Needed Prior to Admission
Prior to admission, international applicants must complete and submit a Declaration and Certification of Finances form. Also, all students must also have uploaded official transcripts and proof of degree – per University Policy these must be verified by the Graduate College prior to admission.

In the on-line application under “Planned Course of Study” please indicate your desire for our graduate program, which is listed in the drop-down menu as Ph.D. Pharmacy.
Applicants must follow instructions on the UIC Graduate College application and submit all requested documents and forms (see below). All applicants are required to upload their documents and academic credentials electronically to satisfy both the program-specific requirements and the admission/institutional requirements. The completed application form and all related materials, including payment of application fees, must be submitted on-line before your application can be considered.
