How To Apply
In order to be part of the UPHARM Program, students must be admitted into the UIC College of Pharmacy Doctor of Pharmacy program and complete the following steps.
- Complete UPHARM application, including three letters of recommendation from people that can speak to your involvement in your community
- Interview with the Urban Pharmacy Recruitment and Retention Committee and receive favorable recommendation
- Pledge to participate fully and complete the UPHARM program and practice pharmacy in an urban Illinois community
- Want to train in an inter-professional environment with other healthcare disciplines to improve the health of urban patients
Please note: All materials must be submitted directly to the UIC College of Pharmacy at the address below by 5:00pm on due date for that particular year.

All complete UPHARM Program applications will be screened by the UPHARM Recruitment and Retention Committee. A select number of applicants will be interviewed and those who are chosen for the UPHARM program will be notified of their acceptance prior to the UPHARM Program Student Orientation.
